The box of this game has the title "Blade Eagle 3-D".

Level Select

For a level select, press DOWN, RIGHT, UP, LEFT, then DOWN at the title screen. If done correctly, a small white number will appear to the right of the word "Eagle", which corresponds to the starting level. Select a level from 0 to 8.
